Health Insurance for People with High Blood Pressure

What to look for in a health plan when you have hypertension.

Hypertension and ACA Protections

High blood pressure (hypertension) is the most common chronic condition in the United States, affecting nearly half of adults. Under the ACA, insurers cannot deny coverage or charge higher premiums based on hypertension or any other pre-existing condition. This protection applies to all ACA-compliant individual and employer health plans.

What Your Plan Should Cover for Hypertension

  • Blood pressure medications: ACE inhibitors, ARBs, beta-blockers, calcium channel blockers, diuretics — most are generic and inexpensive, but verify your specific medication is on the formulary
  • Preventive blood pressure screening: Annual blood pressure checks are a covered ACA preventive service at $0 cost-sharing
  • Primary care visits: Regular monitoring with your primary care physician is the foundation of hypertension management — check copay/coinsurance for primary care visits
  • Cardiology: If you have hypertension with cardiac complications, verify cardiologist access in-network
  • At-home monitoring devices: HSA funds can be used for blood pressure monitors purchased for home use

Hypertension and Medication Costs

Many first-line hypertension medications are available as inexpensive generics. Lisinopril, amlodipine, losartan, and hydrochlorothiazide are all generic and cost $4–$15/month at most pharmacies with GoodRx or similar discount programs, sometimes even without insurance. If your hypertension is well-controlled on inexpensive generics, the medication cost may not drive your plan choice significantly. However, if you take newer brand-name medications or have added cardiac medications, the formulary matters more.

When Hypertension Complicates to Heart Disease

Uncontrolled hypertension can lead to heart disease, stroke, kidney disease, and other complications. If you have hypertension with any cardiac complications, consider a plan with:

  • Cardiologist coverage in-network
  • Strong hospital coverage for potential cardiac events
  • Lower out-of-pocket maximum to cap costs if a major event occurs
